1. Obtain an Application Form

   • Pick up the form from the Division of Passports and Visas, Ministry of Foreign Affairs
   • Or download application form.

2. Complete the Application Form

   • Fill in all necessary information accurately.

3. Submit the Completed Application Form and Required Documents

   • Submission hours: 8:30 a.m. – 1:00 p.m., Monday to Friday only.

4. Schedule Applicant Interview

   • Interview date is provided upon submission of the completed form and required documents.

5. Interview Days

   • Adults (18 years & above): Mondays, Wednesdays, and Fridays
   • Children (17 years & below): Tuesdays and Thursdays
     • Children must be accompanied by parent(s) or legal guardian(s)

6. Make Immediate Payment After Interview

   • After the interview, obtain a Bank Payment Slip (BPS) and make the payment immediately.

7. Passport Fee

   • US$50.00 (paid at the one-stop window in the Division of Passports/Visas)

8. Passport Collection

   • Ready for pickup within 7–10 working days from the date of payment.

Please report anyone soliciting additional fees.
Any applicant paying additional charges does so at their own risk.

Required Documents

Submit one of the following, along with a passport-size photo:

• Birth Certificate
• Old or Existing Passport
• Travel Certificate (TC) or Laissez-Passer
• Certificate of Citizenship or Naturalization
• Court Decree of Oath of Allegiance
